In case you missed filling out the paper reflection survey at the UNITY Midyear Conference closing ceremony on Friday, January 5, please take a few minutes to complete our survey online.
We use your responses collected to help plan our future events! Once you start the survey, you should plan for 15 minutes to complete. Click “next” at the bottom of each page to save your answers and click “done” at the end of the survey to submit and save all your answers online.
Your submission will be completed by clicking “done”, at which point you will be re-routed to the UNITY website.
