Advisors, please download the attached forms. The Medical Authorization and Liability Release form must be signed by a parent or legal guardian if the participant is a minor. Keep the completed form with you during the conference.

The On-Site Registration form is only for those registering at the conference. This form includes signature lines for: Commitment to Participate and Liability Release, Medical Authorization and Liability Release, and Photographic Release. The conference is expected to have the largest number of attendees in UNITY’s history, so to save time, please complete before starting the on-site registration process.
